CHANGE MY PERSONAL DETAILS?

Please let us know of any change of address or telephone number. If you need to change your surname you are required to complete the appropriate forms which will be forwarded to the Central Services Agency; for further details please ask at reception. If you leave the practice area (details available at reception) you will need to register with another doctor. You can do this by contacting your nearest surgery.
